Chronic Disease Management

For patients with chronic diseases such as heart disease, diabetes, thyroid complaints and epilepsy to name but a few, it is important to monitor your condition and therapy by having regular blood tests that may be annual or more frequent. In addition, some of these conditions, including asthma, require assessment by the medical or nursing staff to ensure your condition is properly managed.

For this reason, we will be issuing blood test forms along with your repeat prescriptions wherever possible. It is in your interest to ensure that you arrange to have these vital tests either here at the surgery or at the hospital pathology laboratory.

The reception staff will be encouraging you to make an appointment for your chronic disease check up, where appropriate, when you collect your prescriptions. For patients with more stable conditions, we will be contacting you by telephone or e-mail to complete a questionnaire to help us verify that your condition is satisfactory and stable.

Finally, for patients with diabetes, you will be given a form to encourage you to attend an eye screening test. This test is FREE and it is important for you to have this test even if you do not wear spectacles or believe you have a visual defect. Similarly, you will need an annual urine test to exclude minute traces of protein in your urine - vital for us to ensure that early detection of kidney disease is not missed.
