Personal Medical Services

The Purpose of PMS is to provide a more flexible approach in primary care services and improving patient access to achieve that aim.

Lister Medical Centre became a fourth wave PMS site from the beginning of October 2002. The aim of this change is to give a better, more efficient service to our patients. We aim to provide quicker access to surgery facilities but also intend to provide both a greater range of services as well as improving existing ones.

Patients who wish (with urgent problems), will have the same day access to either a doctor or nurse practitioner, depending on the need. In addition, patients will have the opportunity to see a doctor with a wait no longer than two working days (obviously patients will be allowed to make appointments further apart if they so choose). Naturally, patients with urgent problems will be seen on the day as usual.

In addition, we intend to improve our care in number of key areas. All routine GP appointments will be ten minutes, if needed.

With the increased resources available, the partners will have the time to spend caring for those patients with chronic illnesses.

We intend to target patients with:

  • Coronary Heart Disease (& primary prevention)
  • Stroke (& primary prevention)
  • Heart Failure
  • Asthma
  • Diabetes
  • Chronic Obstructive Pulmonary Disease
  • Cancer
  • The over 75's (& their carers)
  • Mental Health Issues
  • In addition we are also targeting osteoporosis
  • We intend to identify, scan and monitor "at risk" patients.
  • Patients can expect to receive life style advice and encouragement to change where indicated.
