Where You Can Register

You can join our GP surgery if you live in the blue area shown on the map above.

How to Register

You can register online here. It’s easy and takes about 15 minutes.

If you prefer, you can fill out a GMS1 registration form, then call us to book a meeting with our Registration Team.

What to Bring

  • If you have a medical card from your old doctor, bring it with you.
  • If you take regular medicine, please bring your repeat prescription form.

You don’t need ID to register, but it helps if you have one of these:

  • Passport
  • Birth certificate
  • HC2 certificate
  • Badge for rough sleepers
  • Letter from a hostel or mail-forwarding service

If you don’t have a home, you can use a friend’s address, a day centre, or our surgery’s address.

Health Check

We’ll offer you a health check after you register.

If you’re not at home and feel sick, or if you don’t have a GP, you can visit a local doctor for emergency care for up to 14 days. After that, you’ll need to register as a temporary or permanent patient.

You can be a temporary patient for up to 3 months.
This means you can get help from the local GP but still stay with your usual doctor. After 3 months, you’ll need to register again or become a permanent patient.
To register as a temporary patient, just contact the GP practice.

Please note:
The GP practice doesn’t have to accept you as a temporary patient.
But they must help you in an emergency. You can’t register as a temporary patient in the same town or area where you already have a GP.

You can get temporary registration forms from reception.

Your Named Doctor
Everyone who joins Lister Medical Centre gets a named doctor. This doctor is in charge of your care and support.
Your named doctor is also called your allocated GP. They help look after your health.
You can still choose to see any doctor at the surgery, just like before.

Not Sure Who Your Doctor Is?
If you don’t know who your named doctor is, just ask at reception.
